You must have the Adobe Acrobat Reader plug-in for your Web browser to access and print out the below forms. (All Technology Center computers have this plug-in.) If you do not, you can download it free of charge from the Adobe Web site by following the link for Plug-Ins and Updates.
After printing out and filling out these forms, submit them to the Lab Assistant desk in the Technology Center.
